Membership Fee Monthly Fee: Limited time $50 per month until May 4th, 2026 Includes: Up to 3 telehealth visits per month Payment Method: [Auto-pay via card on file] Billing Cycle: Monthly (auto-renewal unless canceled) Services INCLUDED in Membership Virtual urgent care visits Preventive care counseling Chronic disease management (e.g., hypertension, diabetes follow-up) Mental health visits (non-emergency) Birth control counseling/prescriptions Weight management consultations Medication refills (at provider discretion) Basic medical advice and care coordination Disability or work/school notes (when appropriate) Services NOT INCLUDED in Membership Membership does NOT include: Emergency care (call 911 or go to ER) In-person visits or procedures Laboratory testing fees Imaging (X-rays, CT scans, MRI, etc.) Specialist referrals costs Medications or pharmacy costs Controlled substance prescriptions (if applicable per policy) Wound care procedures requiring in-person treatment Surgical procedures or invasive treatments Nature of Telehealth Services Services are provided virtually only Care is limited by the nature of telemedicine Not all conditions can be diagnosed or treated via telehealth Provider may recommend in-person evaluation when necessary Not Insurance This membership is NOT health insurance It does not replace comprehensive medical coverage Patients are encouraged to maintain insurance for: Hospitalizations Emergencies Specialty care Membership includes up to 3 visits per month Unused visits do not roll over Prescription Policy Prescriptions are provided at the provider’s discretion No guarantee that medications will be prescribed Controlled substances may be restricted per state/federal law By enrolling you agree to: Provide accurate medical history Use services appropriately Seek emergency care when needed Follow medical advice provided Cancellation Policy Cancel by calling us before next billing cycle No refunds for partial months Membership continues until canceled Refund Policy: Membership fees are non-refundable Exceptions may be made at provider discretion Communication may occur via video, phone or text.
